Sync re-adds a track if there are duplicate track names.
Some albums have two or more tracks with the same name. For example The Beatles Anthology 2 (CD 1) has track 7 named Yesterday and track 11 named Yesterday.
When you first sync your iTunes library to your mobile device, all of the tracks sync correctly."
The message is "Sync completed correctly". The device is in sync and no files were transferred."
If you then click Sync again it will say "1 song added" and examining that message lists it as "Yesterday, The Beatles".

The only way I've found to get around the bug is to un-sync the problem album. Change the track name in iTunes. Do a Library Cleanup from the mobile device settings menu. Choose "Reset sync info" from the doubleTwist Sync Debug menu and then resync the iTunes library.
On Windows 10 with doubleTwist Sync 4.0.4.19779 and iTunes 12.9.4.104
